Medical Waiver & Policies

INSURANCE: It is the responsibility of every individual, their parents or legal guardian, to provide for their own accident and health coverage while participating in all the ME…Volleyball Academy Inc. activities. ME…Volleyball Academy Inc. does not provide any such coverage for its participants.

AS A CONDITION FOR PARTICIPATION IN ACTIVITIES OFFERED BY ME…VOLLEYBALL ACADEMY INC. THE UNDERSIGNED HEREBY ACKNOWLEDGEMENT AND AGREES AS FOLLOW: (1) PARTICIPANT ENGAGES IN ALL ACTIVITY WITH FULL KNOWLEDGE OF POSSIBLE RISKS. THERE ARE CERTAIN RISKS INHERENT TO PARTICIPATION IN THE VOLLEYBALL CLASSES AND SPECIAL VOLLEYBALL EVENTS WHICH CAN RESULT IN INJURY TO THE PARTICIPANT. (2) PARTICIPANT, PARTICIPANT’S PARENTS/GUARDIAN, SIBLINGS, OTHER FAMILY MEMBERS AND VISITORS HEREBY ACKNOWLEDGE THAT RISKS OF INJURY ARE INHERENT AND HEREBY WAIVE ON BEHALF OF PARTICIPANT, THEMSELVES, MINOR SIBLINGS, OTHER FAMILY MEMBERS AND VISITORS. THEIR HEIRS SUCCESSORS OR ASIGN, ANY AND ALL CLAIMS IN TORT OR FOR CIVIL LIABILITY AGAINST ME…VOLLEYBALL ACADEMY INC. ARISING FROM THE FORESEEABLE AND/OR CUSTOMARY RISK OF THE SPORT (VOLLEYBALL) OR INTENDED ACTIVITY. (3) THE UNDERSIGNED HEREBY STATES THAT PARTICIPANT HAS NO MEDICAL OR PHYSICAL CONDITIONS WHICH WOULD PREVENT PARTICIPANT FROM FULLY PARTICIPATING IN ALL NORMAL AND CUSTOMARY ACTIVITY OF THE SPORT (VOLLEYBALL) AND AGREE TO IMMEDIATELY NOTIFY ME…VOLLEYBALL ACADEMY INC. IN WRITING IF SUCH MEDICAL OR PHYSICAL LIMITATiONS, NOTABLE ILLNESS, INJURY, CONDITION, ANY INTOLERANCE TO FOOD, DRUGS AND MEDICATION OCCURS.

I WILL BE RESPONSIBLE FOR MY CHILD MEDICAL COST DUE TO AN ACCIDENT, INJURY OR ILLNESS. I WILL HOLD ME…VOLLEYBALL ACADEMY INC, DIRECTORS, ALL EMPLOYEES, VOLUNTEERS, OFFICERS AND OTHER AGENTS HARMLESS FOR INCIDENTS WHICH MAY ARISE FROM PARTICIPATION IN THE VOLLEYBALL CLINICS AND ACTIVITIES. I FULLY UNDERSTAND THAT ME…VOLLEYBALL ACADEMY INC. DOES NOT EMPLOY A MEDICAL PRACTITIONER OF ANY KIND. I HEREBY RELEASE ME…VOLLEYBALL ACADEMY INC. TO RENDER FIRST AID TO MY CHILD IN THE EVENT OF ANY INJURY OR ILLNESS AND IF DEEMED NECESSARY TO CALL AMBULANCE FOR WHICH I AGREE TO PAY. I UNDERSTAND THAT I AM RESPONSIBLE FOR MY CHILD, MYSELF, MY FAMILY MEMBERS, FRIENDS OR ANY OTHER GUEST BEHAVIOR AND SAFETY WHILE ON THE FACILITY AREAS INCLUDING FRONT DESK, RESTROOMS, WAITING AREAS, PARKING LOTS, PLAY YARD, ETC. I UNDERSTAND THAT OTHER UNREGISTERED CHILD IN MY CARE ARE NOT ALLOWED BEYOND VIEWING AREAS OR ON/NEAR ANY CLASS COURTS. I UNDERSTAND THAT EVERY GUEST ON MY PARTY IS RESPONSIBLE FOR THEIR OWN MEDICAL COST DUE TO AN ACCIDENT, INJURY OR ILLNESS WHILE ON THE ME…VOLLEYBALL ACADEMY AREAS. I WILL HOLD THE ME…VOLLEYBALL ACADEMY INC. AND ITS DIRECTORS, OFFICERS, EMPLOYEES, VOLUNTEERS AND OTHER AGENTS HARMLESS FOR INCIDENT WHICH MAY ARISE FROM THE PARTICIPATION IN THE VOLLEYBALL COURT AND OTHER AREAS ON THE FACILITY, REALIZING THAT THERE ARE RISK IN THIS ACTIVITIES. I UNDERSTAND THAT NO INAPPROPRIATE LANGUAGE OR PROFANITY IS ALLOWED IN THE FACILITY. THE FACILITY IS A NO SMOKING PLACE, NO ALCOHOL BEVERAGE, NO GUNS, KNIVES OR ANY OTHER WEAPON, NO OUTSIDE BALLS, NO OUTSIDE FOOD, NO PETS ALLOWED. I GIVE PERMISSION FOR SOCIAL MEDIA NETWORKS PHOTOS, PHOTOGRAPHS, VIDEO TAPE TO BE MADE OF MY CHILD TO BE USED SOLELY FOR PUBLICITY AND TRAINING PURPOSES BY ME…VOLLEYBALL ACADEMY INC. I GIVE PERMISSION TO SUBSCRIBE ALL EMAIL ADDRESSES PROVIDED ON THE ME...VOLLEYBALL ACADEMY REGISTRATION FORM TO BE USED FOR ADVERTISING, PROMOTE PRODUCTS OR SERVICES. ME…VOLLEYBALL ACADEMY INC. ENSURE THAT ALL PERSONAL INFORMATION COLLECTED ON INDIVIDUAL AND CHILDREN WILL BE CONFIDENTIAL.

ME…VOLLEYBALL ACADEMY POLICIES: It is very important that each player arrives on time to the clinic, the first minutes of each clinic are used to warm up and stretch. Being late to the clinic could result in an injury. *Be aware that players are not allowed to step on court until coaches advise them to do so. The use of mask is recomended for all unvaccinated visitors.

All members (players) must wear the ME... Volleyball Academy Inc. t-shirt and knee pads, tennis shoes, volleyball shorts, spandex or any other sport pants. We are not responsible for any injuries caused due to the athlete not wearing knee pads. Please remove all jewelry prior to any volleyball event. No outside food allowed, only water or sport drink. Also, please keep personals balls (volleyballs, basketballs, footballs, etc) in your vehicle as the only ones permitted for use are the ones provided during any of our sessions.

ME… VOLLEYBALL ACADEMY INC. NO seating will be provided for parents/guardians so feel free to bring your own chair. ME... Volleyball Academy Inc. will not be responsible for players or other children after clinics or any other events. All players and parents/guardians, siblings, family members and visitors are responsible for their personal items.

This facility is a NO Smoking place, NO alcoholic beverages, NO guns, knives or any other weapon, NO outside balls, NO outside food other than water or any sport drink. Gum is NOT allowed out on the gym floor. NO pets allowed except for service animals assisting a person with a disability. Please only sit down in the designated areas. NO children allowed to play in play yard areas. Please do not park in handicapped parking without handicap license plates or tag. NO sitting will be provided for parent/guardian feel free to bring your own chair. No one can play with or hang on any of the basketball hoops. If anyone damages the basketball hoops, you will be charged $200.00. ME Volleyball Academy and Forest Hills Presbyterian Church are not responsible for valuables or belongings that may be stolen or damaged in your vehicle.

This facility is a NO Smoking place, NO alcoholic beverages, NO guns, knives or any other weapon, NO outside balls, NO outside food other than water or any sport drink. Gum is NOT allowed out on the gym floor. NO pets allowed except for service animals assisting a person with a disability. Please only sit down in the designated areas.
